8/11/2023 EnchantaVaughn Approved 1.Behavior: Ben-Nita greeted QMHP-C by saying good afternoon and hello. Ben-Nita was observed sitting quietly on the couch displaying positive prosocial behaviors during quarantine. Ben-Nita displayed productive behaviors and was very persistence throughout the day. Ben-Nita appeared to be in an excited and upbeat mood as evidenced by her smiling and initiating a conversation with QMHP-C. 2. Behavior: Ben-Nita presented within drawn behaviors during the second portion of the session when discussing her behavior about respecting those who are in authority. 1.Intervention: QMHP-C had one on one with Ben-Nita. QMHP-C will conduct evening session to debrief and talk about a plan to follow and ways to reduce tension and negative energy so that she can have a successful and positive day. QMHP-C provided support and encouragement with Ben-Nita staying on task in the home. QMHP-C will educate Ben-Nita about being respectful, encouraging, mindful, and fair to peers and authority while making good decisions. QMHP-C will teach Ben-Nita necessary coping skills to decrease impulses. QMHP-C will brainstorm strategies with Ben-Nita and encourage self-monitoring. QMHP-C will provide mediation and relaxation techniques, as well as, simple praises to help build high self-esteem and challenge Ben-Nita to do good on her own. 1.Response: Ben-Nita displays normal behavior in home setting. Ben-Nita exhibited a positive attitude toward mother and counselor. Ben-Nita said she is having challenges with certain activities and needs help. Ben-Nita said she was being good but got in trouble for talking back to her mom. Ben-Nita knows what is expected of her and is trying to make better choices. Ben-Nita verbalizes positive feelings about herself but recognizes areas she needs to work on. Ben-Nita smiled and hugged QMHP-C for being there for her when she needed. Ben-Nita displayed positive interaction with her peers and was supported with building healthy relationships in other group settings. Although Ben-Nita has difficulty staying focused, she is extremely dedicated to getting her chores done on time and getting her daily free time. Ben-Nita presents a positive attitude when given the opportunity to earn a reward. Ben-Nita implemented the positive reinforcement provided by her mother and was able to stay motivated and enjoy her session. 2.Response: Ben-Nita exhibited good manners after earning incentive for her hard work. Ben-Nita transitions smoothly to the next activity, displaying appropriate behaviors. Ben-Nita expresses good statements about her day saying she was doing her work, using her manners, and being fair to others. Ben-Nita is more confident, as evidenced by assertive and proud tone. Ben-Nita have been doing a better job controlling impulsive and hyperactive behaviors, due to keeping her hands to herself and complying to home rules. Ben-Nita reduces sadness and finds more excitement in learning about different ways to control her reaction to certain situations and be more receptive and open in therapeutic setting. Ben-Nita practiced some of the examples of alternate behaviors (e.g. coloring or drawing, thinking about happy moments, repeating positive “I-statements” 5-10 times, and asking questions that will help her understand things better) when she found herself bored or unproductive. Ben-Nita displays more productiveness and is observed paying more attention and being respectful to authority. 2. Intervention: QMHP-C inquired about Ben-Nita’s day and what things is she doing to make it a good day, as well as, a productive day. QMHP-C educated Ben-Nita about what she can do to make social distancing fun and interesting, such as reading her favorite book, drawing in a journal, or simply closing her eyes and practicing mindful exercises, such as deep breathing and positive self-talk or even taking a nap and getting much needed rest. QMHP-C prompted Ben-Nita to refrain from impulsive, hyperactive, defiant, and aggressive behaviors especially running, refusing authority instruction, and interrupting another’s person day. QMHP-C utilized behavior modification strategies including removing Ben-Nita from the stimulant environment in efforts for her to reduce inappropriate behaviors. QMHP-C will monitor effects of meditation and coping practices daily and report progress to mom.
